fibUK seminar: Learning from Failures

This seminar will present accounts of recent failures and discuss their likely
causes based on the evidence publicly available and the lessons to be learned
for designers and constructors. It will discuss our failure to learn from some well
documented recurring problems, and attempt to propose solutions for reducing
this corporate amnesia.

The seminar is suitable for those at all stages of their career in design,
construction and academia and will promote:
  • learning from real, recent failures
  • an awareness of common problems in design and construction and how to eliminate or mitigate them
  • improved methods for capturing and cascading such knowledge

In the past 3 years a number of highway bridges have collapsed in Italy, killing 46 people. This presentation will give a brief overview of the collapses and promote discussion on the challenges faced in keeping bridges safe during their serviceable life.

In 2018, this incomplete bridge collapsed - killing six people and injuring many others. This presentation aims to provide a description of the structure and share the up to date findings of the US National Transportation Safety Board’s investigation.

Many failure types that occur in design and construction have happened before, and often stem from well known causes. These repeat mistakes can occur because an organisation has never encountered the problem, or because they have failed to retain previous learning. This talk will highlight recurring problems in the design of prestressed and reinforced concrete structures, and will propose solutions for preventing corporate amnesia.
